Iowa’s count is still rising mainly because recalls do not stop cases that were already set in motion. Cyclospora symptoms usually appear about a week after exposure, but can take up to two weeks or more, and CDC says confirming and linking a case to the outbreak can take as long as six weeks. That means people exposed before recalled lettuce was removed may only now be getting sick, getting tested, and being added to the official tally. The Iowa total also is not limited to the recalled iceberg lettuce outbreak. State officials say 47 Iowa patients had recently traveled outside the United States, and those infections were likely acquired abroad. Federal and state investigators have also warned there may be multiple Cyclospora outbreaks or sources this season, not just the iceberg lettuce tied to Taylor Farms de Mexico and Taco Bell-linked illnesses in nine states. Another reason totals keep climbing is that source tracing is slow. Patients are being interviewed about everything they ate in the two weeks before illness, but Cyclospora’s delayed onset makes food recall difficult and traceback complicated, especially with fresh produce moving through long supply chains. So a rising case count does not necessarily mean recalled lettuce is still being sold. It more often reflects reporting lag, delayed illness onset, ongoing lab confirmation, travel-related infections, and the possibility that Iowa cases include exposures beyond the recalled product.
