Probably not on its own. Even if watchOS 27 improves Apple’s hypertension notifications, there is no evidence yet that it will turn them into a reliably standalone warning system. The key issue is sensitivity. Apple’s published validation data for the current feature shows it alerts only a little over 41% of people with undiagnosed hypertension. That means most true cases could still be missed. A later JAMA modeling study suggested an alert is meaningful when it appears — about 69.1% of alerted users were estimated to actually have hypertension — but a missing alert does not safely rule the condition out. In that analysis, roughly 21% of users without an alert could still have hypertension. That makes the feature useful as a screening prompt, not a dependable all-clear. It is also not a blood-pressure monitor: the Apple Watch does not provide systolic/diastolic readings in mmHg and cannot replace a validated upper-arm cuff. watchOS 27 may improve reliability if Apple has refined the algorithm, expanded the data it uses, or added better clinical validation. But Apple has not disclosed what is changing, and the reported upgrade is still under FDA review. Until those details and performance numbers are public, there is no basis to say reliability has materially improved. For users, the practical takeaway is unchanged: if you get an alert, treat it seriously and confirm with a validated cuff and a clinician. If you do not get one, do not assume you are clear of hypertension.

Apple’s upgraded Apple Watch hypertension notification feature is under FDA review and could arrive with watchOS 27, according to a Digitimes report cited in the latest coverage.
