Penn Study Links 30 Aducanumab Doses to Lower Tau in Cleared Alzheimer’s Brain Regions

A single-patient autopsy found Alzheimer’s brain regions where aducanumab had cleared most amyloid showed little or no tau, while nearby uncleared areas remained packed with tau, inflammation and neurodegeneration.
30 doses over 4.5 years created a rare side-by-side comparison: raised gyri lost most plaques, but adjacent sulci retained abundant amyloid, matching scans that had already shown uneven clearance during life.
JAMA and an Alzheimer’s Association conference report frame the case as unusually direct human evidence that removing amyloid may interrupt the downstream cascade tied to neuron loss and cognitive decline.
The finding remains limited to 1 patient, cannot prove causation, and suggests benefits may take years to emerge—longer than the roughly 18-month follow-up common in major Alzheimer’s drug trials.
Because newer anti-amyloid drugs such as donanemab and lecanemab use the same strategy, researchers say the result could guide efforts to improve regional plaque clearance and test earlier treatment.
