Police said a Toyota Highlander driver suffered a medical episode on the access road, hit a sign, crossed into the highway and struck a Dodge Ram, which then hit a Chevrolet Suburban and rolled several times.
SAPD's preliminary report listed both children's injuries as life-threatening, though officers later told KSAT the 11-year-old's condition had improved; the 15-year-old suffered a broken ankle.
The Dodge Ram's 40-year-old driver suffered non-life-threatening injuries, while the driver who triggered the crash stayed at the scene and cooperated with investigators.
Northbound U.S. 281 was shut down after the crash and reopened around 6 p.m. Saturday as the investigation continued.
