7-methylxanthine, an oral caffeine metabolite, is emerging as a disease-modifying treatment for childhood myopia after Danish clinical and real-world data showed slower progression and lower risk of high myopia.
A retrospective study of 711 Danish children estimated 7-MX reduced progression by 0.70 diopters over three years—about 48% for an 11-year-old—while a 36-month placebo-controlled pilot in 68 children reported reduced axial growth and no adverse effects.
Long-term observational data also linked continuous treatment from age 10 to 17 with markedly less high myopia: among 65 treated children, nine developed it versus 20.3 cases predicted from an untreated Dutch cohort.
The drug targets the biology behind worsening myopia by blocking adenosine A2A receptors, boosting dopamine signaling and strengthening scleral collagen to resist axial elongation—the eye stretching that drives later retinal and glaucoma risks.
With myopia projected to affect nearly 50% of the world and about 1 billion people expected to exceed -5.00 diopters by 2050, a sustained-release version is headed for a global Phase III trial to test whether Danish results hold internationally.
