Among 1,649 anticoagulated atrial fibrillation patients with ischemic stroke, 43.9% had at least one identifiable reason for treatment failure, most often interacting drugs or competing non-cardioembolic stroke mechanisms.
At 90 days, patients with any potential cause faced about double the risk of another ischemic stroke, with an adjusted hazard ratio of 2.04.
Interacting drugs appeared especially tied to heart risk: they were linked to a 3.26-fold higher 90-day myocardial infarction risk, but not to higher recurrent stroke risk.
The findings suggest many breakthrough strokes may reflect alternative stroke pathways rather than inadequate anticoagulation alone, supporting more tailored secondary prevention.
