A four-year American Heart Association program aims to standardize integrated cardiovascular-kidney-metabolic care for more than 250,000 patients at 150 healthcare sites in 15 regions.
Nearly 9 in 10 U.S. adults have at least one CKM risk factor, the group said, yet low awareness and fragmented specialty care leave linked heart, kidney, diabetes and obesity risks untreated until hospitalization.
The initiative is designed to give clinicians evidence-based tools and cross-specialty workflows, including the PREVENT risk calculator, Life's Essential 8 guidance and a CKM implementation guide.
DaVita, a sponsor of the effort, said integrated care can cut downstream complications and highlighted AI investments that have reduced heart-failure readmissions by 50%.
