Zakrzewska died in hospital on April 25, prompting police to upgrade the case from attempted murder; Chyche, a bouncer in his 50s, suffered life-changing injuries.
Carrington, appearing by video link from Bronzefield prison and representing herself, was not asked to enter a plea on a third charge of drink-driving after allegedly recording 61 microgrammes of alcohol per 100ml of breath.
A pre-trial hearing is set for August 11 and October 9, with Carrington's trial due to begin in January.
